The Case Management Credo: Coordinate Advocate Navigate
Members of the National Case Management Network of Canada (NCMN) believe the practice of Case Management is a vital and dynamic strategy that assists in the delivery of care efficiently, effectively, and holistically. While fortifying a case manager society that supports and sustains all providers of Case Management, NCMN’s goal is to unify Case Management providers into an interdisciplinary association of providers who develop, collect, implement, and disseminate a uniform knowledge base and best practices in Case Management.
Members of NCMN believe an interdisciplinary case management society can produce better health care, improved access to services, and more efficient use of resources resulting in better satisfaction for the people of Canada and more equitable uptake of health care dollars.
